Abstract
Background: Coronary Artery Disease (CAD) is very common and remains an important cause of mortality accounting for 20-25% of all mortality in public hospitals in Malaysia. Coronary artery bypass grafting (CABG) is one effective mode of treatment for patients with CAD. However, CABG operation is associated with high red blood cell (RBC) transfusion that in turn associated with various morbidity and mortality. This study aims to find the association of RBC transfusion with morbidity and mortality in post CABG patients in National Heart Centre, Kuala Lumpur (IJN). Methods: Retrospective Cross Sectional Study was performed with data from 434 patients who underwent CABG over a period of two years (2013 and 2014). Data were retrieved from the computerized patient data system of IJN. Study subjects selected by systematic random sampling every fifth subject of the patient's medical record in the sequence of dates of the year, after data cleansing performed. Data retrieved were filled in the proforma. All demographic and clinical data in regards to outcome ofRBC transfusion- mortality and morbidity were captured. Results: 64.3% of CABG patients (n=279) received RBC transfusion perioperatively while 35.7% (n=155) did not. Age, gender, Body Mass Index (BMI) and Ejection Fraction (EF), were 4 factors that significantly contributed to RBC transfusion. RBC transfusion in turn, shown to be significant contributor for longer ICULOS and HLOS, but not for infection and death risk. Multiple logistic regression revealed that patients were 3.5% more likely to be transfused for every I year increase of age, while increase in 1 kg/m2 of BMI and 1% of EF will reduce the odds ofRBC transfusion by 13.0% and 3.0%. Conclusions. Age, gender, BMI, and EF are four important determinants for probability of RBC transfusion in CABO while RBC transfusion will result in longer ICULOS, HLOS. Probability of RBC transfusion will be higher with those of older age and reduced with increase in BMI and EF.
